On Tuesday, May 14th, Oak Park High School will be hosting a FREE sports physical day in the Main Gym from 9:00am - Noon, for all current OPHS enrolled students who plan to participate in OPHS activities for the 2019-2020 school year.

Informational Packets including the physical and concussion forms can be located outside of the Activities Office, and will need to be filled out and signed by Parents and Participants before getting the physical.  It is important that all forms are filled out and signed and parents and students need to be sure to concentrate on Pages 1, 3, and 4 of the physical form.

Students will be call down alphabetically during the day on May 14th, and will need to bring paperwork with them at that time.  Once the physician has completed the physical, the student will turn the paperwork into the Athletic Office immediately following the exam.
